Detail report card for one student
Posted May 13th, 2007 by peggyThe attached document is an example of a detailed report card which accompanies the summary reportcard also available on this site. Each class listing includes a summary of the course material covered, the outcomes for the term, and the ratings given by the teacher for them. In addition, the system enables the teacher to attach a narrative which allows for more detailed comments from the teacher.

Summary Report Card
Posted May 13th, 2007 by peggyThis document is the summary page for a typical report card using EASE. It lists each class, and the student's percent proficient or better and percent high performance ratings for the outcomes evaluated in that class. This reportcard is accompanied by a detailed listing for each class, showing each outcome and the student's rating for it, and giving the teacher an opportunity to add a narrative for further depth.

Sample 7th Grade Humanities Outcomes, Rubrics, Evidence

Sample Evidence Worksheet Showing Relationship Between Evidence, Outcomes
Posted April 1st, 2007 by peggyThis worksheet gives visual cues for how each piece of evidence (quizzes, Problems of the Week, classroom observation, homework, etc.) relates to one or more outcomes. The completed grid gives the teacher a handy way to see whether a student is proficient, high performing, or not yet proficient for each outcome. Shaded areas denote assignments, tests, etc. that do not apply to the given outcome. A teacher would review the general trend of a student's performance for the outcome under consideration as the basis for making a final determination.
